The registration of FOMEMA Medical Examination can only be made by the Authorized Employers and Agents of the Foreign Workers.  

In the condition that registration is made by the representative, please provide an authorization letter and copy of NRIC of representative.

  1. Copy of your NRIC for Malaysian / Copy of your passport if you are non-Malaysian.
  2. Copy of working permit/ Calling VISA/ Maid Online Slip/ Recalibration approval slip by the Immigration Department of Malaysia. 

Document required: Company Employer 

  1. Copy of your company’s registration documents (Form 9 or equivalent).
  2. Copy of the NRIC/Passport of contact person.
  3. Copy of the letter authorizing the contact person to perform worker registrations.
  4. Copy of working permit/ Calling VISA/ Maid Online slip/ Recalibration approval slip by the Immigration Department of Malaysia.
