Step 1: You need to refer to the registered clinic to obtain the appeal form
- Employer: Appeal form appendix 4 and 6
- Doctors: Appeal form appendix 1 and 5
Step 2: Submit the appeal form to your examine clinic within 14 days from the certification date
Step 3: Refer to your clinic for appeal instruction
Step 4: Clinic submit document and report to FOMEMA
Step 5: Check appeal result

How to check your worker’s appeal status?
Step 1: Login to https://portal.fomema.my/users/sign_in
Step 2: Go to appeal tab
Step 3: Search using workers name, worker’s code, appeal status or appeal date
