For registration you may walk-in and bring along below document:

1. Registration Form (https://uat.fomema2u.com.my/forms/)
2. Original passport
3. Copy of original passport
4. Copy of working permit/ RTK approval slip/ SMO approval slip
5. Payment MYR180 male/ MYR190 female via bank draft / CIMB clicks / QR Pay (DuitNow) (Provide link on topic Learn more on what are the payment method accepted)
6. Authorisation letter for representative only
7. Copy of NRIC of employer/ representative
